THE DAKOTA CENTER FOR INDEPENDENT LIVING BELIEVES IN EMPOWERING PEOPLE WITH DISABILITIES BY MINIMIZING BARRIERS, ADVOCATING FOR INCLUSION, AND PROMOTING CONSUMER CHOICE.

Part 3 - Line 4a

DEPARTMENT OF HEALTH AND HUMAN SERVICES GRANT - SERVES 18 COUNTIES IN SOUTH CENTRAL NORTH DAKOTA. SERVED 504 CONSUMERS IN DIRECT SERVICE, THIS INCLUDES THE SOUTH WEST COUNTIES IN NORTH DAKOTA. PROVIDED 2,484 INFORMATION AND REFERRALS FOR INDIVIDUALS. ASSISTED 68 INDIVIDUALS WITH ENROLLMENT/COMPARISON IN MEDICARE PART D PLANS. MADE SEVERAL TRIPS TO THE STANDING ROCK SIOUX RESERVATION PROVIDING MONEY MANAGEMENT CLASSES AND DRIVERS PERMIT CLASSES TO 125 STUDENTS. ASSISTED CONSUMERS WITH TAX RETURNS AND WITH THE NORTH DAKOTA RENTAL REBATE. PROVIDED TECHNICAL ASSISTANCE AND/OR ADVOCATE FOR ACCESSIBLE CONSTRUCTION AND RENOVATION OF BUSINESSES. PROVIDED DEMONSTRATIONS ON ASSISTIVE TECHNOLOGY IN VARIOUS COUNTIES. PROVIDED ASSISTANCE TO CONSUMERS AFFECTED BY COVID-19 PANDEMIC.


MFP GRANT - TRANSITIONED 43 INDIVIDUALS FROM NURSING FACILITIES BACK INTO THE COMMUNITY. ASSISTED IN PURCHASING HOME FURNISHINGS SUCH AS BEDS, SHEETS, BLANKETS, PILLOWS, FOOD, DISHES, KITCHEN TABLE AND TV'S. ALSO PROVIDED ASSISTIVE DEVICES SUCH AS LIFT CHAIRS, THREE WHEEL SCOOTER, SET BACK HINGES, SHOWER CHAIR, HOYER LIFT, NON SLIP MATS, REACHES, RAMPS AND MODIFIED EATING UTENSILS.


STATE GRANT - SAME ACHIEVEMENTS AS IN 4(A) ABOVE. THE SERVICE AREA FOR THIS GRANT CONSISTS OF COUNTIES IN SOUTH WEST NORTH DAKOTA.
