SEMCIL'S PERSONAL CARE ASSISTANT (PCA) PROGRAM IS A PERSONAL CARE PROVIDER ORGANIZATION (PCPO). A PCA FOLLOWS A PLAN OF CARE DEVELOPED BY THE PCA RECIPIENT AND A QUALIFIED PROFESSIONAL (AS DEFINED BY THE MN DEPARTMENT OF HUMAN SERVICES). THE QUALIFIED PROFESSIONAL ENSURES THAT THE PCA SERVICES PROVIDED FOLLOW THE CARE PLAN AND ASSISTS THE PCA RECIPIENT IN EVALUATING THE WORK PERFORMED BY THE PCA. PCAS AND HOMEMAKERS MAY ASSIST WITH ACTIVITIES OF DAILY LIVING, HEALTH RELATED PROCEDURES AND TASKS, INSTRUMENTAL ACTIVITIES OF DAILY LIVING, MEAL PREPARATION AND ESSENTIAL HOUSEKEEPING AND COMMUNITY INTEGRATION.


SEMCIL'S INDEPENDENT LIVING PROGRAM SERVES PEOPLE OF ALL AGES WITH ALL TYPES OF DISABILITIES. SEMCIL PRIMARILY SERVES PEOPLE WITH PHYSICAL, MENTAL HEALTH OR COGNITIVE DISABILITIES, NEURODEVELOPMENT DIFFERENCES, AND TRAUMATIC BRAIN INJURIES. SEMCIL'S INDEPENDENT LIVING PROGRAM ASSIST PERSON WITH DISABILITIES TO GAIN THE SKILLS, TOOLS, AND SUPPORT THEY NEED TO LIVE INDEPENDENTLY. INDEPENDENT LIVING IS A PHILOSOPHY. ONE THAT FOCUSES ON SELF-DETERMINATION, CHOICE, EQUAL OPPORTUNITY, AND ACCESS. THE PERSON WITH THE DISABILITY DEFINES THEIR UNIQUE GOALS AND SEMCIL SUPPORTS THEIR JOURNEY TO COMPLETE THOSE GOALS. SERVICES PROVIDED INCLUDE ACCESSIBILITY SOLUTIONS, ADVOCACY, INDEPENDENT LIVING SKILLS TRAINING, PEER SUPPORT, INFORMATION AND REFERRAL, AND TRANSITION SERVICES.


DISABILITY HUB MN (PREVIOUSLY KNOWN AS DISABILITY LINKAGE LINE DLL) IS A FREE STATEWIDE TELEPHONE INFORMATION AND REFERRAL RESOURCE THAT PROVIDES MINNESOTANS WITH DISABILITIES OR CHRONIC ILLNESSES AND THEIR REPRESENTATIVES WITH A SINGLE ACCESS POINT TO CONNECT TO COMMUNITY SERVICES. THE HUB HELPS DECISION MAKING AND HELPS CALLERS FIND SOLUTIONS. HUB OPTIONS COUNSELORS, WORK AND BENEFITS SPECIALISTS OR COMMUNITY LIVING CONSULTANTS WILL PROVIDE ONE TO ONE ASSISTANCE TO HELP CALLERS LEARN SUPPORTS AND SERVICES THEY CHOOSE. COMMON REQUESTS INCLUDE INFORMATION AND REFERRALS ON DISABILITY BENEFITS PROGRAMS, HOME MODIFICATIONS, ASSISTIVE TECHNOLOGY, PERSONAL ASSISTANCE SERVICES, TRANSITION SERVICES, ACCESSIBLE HOUSING, EMPLOYMENT, SOCIAL ACTIVITIES AND DISABILITY RIGHTS.
