OUTREACH SERVICES TO THE CHRONICALLY HOMELESS AND THOSE NEAR HOMELESS BY PROVIDING FOOD, CLOTHING, AND OTHER BASIC NECESSITIES WHILE ALSO HELPING TO FACILITATE OTHER NECESSARY SERVICES. WE ALSO HELP FACILITATE OTHER PROGRAMS INCLUDING HOUSING NAVIGATION. MCMM DISTRIBUTED SUPPLIES, CLOTHING AND FOOD TO APPROXIMATELY 2,682 INDIVIDUALS. MCMM PROVIDED SERVICES, SUPPLIES AND FOOD 365 DAYS IN 2021 WITH APPROXIMATELY 58,750 ENCOUNTERS WITH INDIVIDUALS.MCMM WAS PART OF 2 CITY OF DETROIT INITIATIVES; THE CITY OF DETROIT DHOT (DETROIT HOMELESS OUTREACH TEAM) PILOT PROGRAM THAT WE PARTNERED WITH THE CITY OF DETROIT'S HOUSING AND REVITALIZATION DEPARTMENT, DETROIT WAYNE INTEGRATED HEALTH NETWORK AND THE DETROIT POLICE DEPARTMENT. THE DHOT PROGRAM IS FOCUSED ON STREET OUTREACH SERVICES ENGAGING CHRONICALLY HOMELESS INDIVIDUALS FOR MENTAL HEALTH SERVICES, SUBSTANCE ABUSE TREATMENT AND HOUSING NAVIGATION. THE SECOND CITY OF DETROIT INITIATIVE IS THE WOODWARD & 8 MILE IMPROVEMENT PROJECT, A PARTNERSHIP WITH OAKLAND COUNTY AND THE CITY OF FERDALE. MCMM IS THE LEAD SOCIAL SERVICES ORGANIZATION FOCUSED ON ENGAGING UNSHELTERED CLIENTS DOWN 8 MILE IN DETROIT WITH HOUSING NAVIGATION, SUBSTANCE ABUSE TREATMENT AND MENTAL HEALTH SERVICES.


TRANSITIONAL SUPPORT SERVICES TO THOSE TRANSITIONING FROM BEING HOMELESS INTO HOUSING AS WELL AS THOSE ALREADY IN HOUSING THAT ARE NEAR HOMELESS. MCMM HELPED FACILITATE NEW HOUSING AND TRANSPORTATION FOR APPROXIMATELY 263 INDIVIDUALS.


MATS & PILLOWS FOR THE HOMELESS IS TO HELP PROMOTE COMMUNITY SERVICE WHILE EDUCATING INDIVIDUALS ON HOMELESSNESS AND NEAR HOMELESSNESS BY RECYCLING PLASTIC BAGS AND MAKING THEM INTO SLEEPING MATS & PILLOWS FOR THE HOMELESS.
