THE WAUKESHA FREE CLINIC, INC. PROVIDES HEALTHCARE SERVICES TO THE UNINSURED, UNDERINSURED, AND LOW-INCOME FAMILIES AND INDIVIDUALS WHO LIVE, WORK, OR ATTEND SCHOOL IN THE WAUKESHA COUNTY COMMUNITY, WITH A HOUSEHOLD INCOME CRITERIA OF 300% OF FEDERAL POVERTY LIVING GUIDELINES (FPL).THE WAUKESHA FREE CLINIC FOLLOWS A CONTINUITY OF CARE MODEL WHICH INCLUDES: PHYSICAL EXAMS, NURSE TRIAGE, PRESCRIPTION MEDICATION ASSISTANCE, WALK-IN SERVICES, COURTESY VISITS, PHYSICAL THERAPY, PANDEMIC-RELATED SERVICES, AND MORE.THE WAUKESHA FREE CLINIC OFFERS HOMELESS OUTREACH AND HISPANIC HEALTH OUTREACH PROGRAMS.
