TO ENSURE EQUAL ACCESS TO QUALITY DENTAL CARE AND PREVENTATIVE EDUCATION.

WE PROVIDE COMPREHENSIVE DENTAL SERVICES TO PEOPLE IN NEED OF ALL AGES IN OUR FOUR CLINICS LOCATED IN SAINT PAUL, MAPLEWOOD, ROCHESTER AND ROBBINSDALE. IN 2021, WE PROVIDED 90,811 PATIENT VISITS IN OUR CLINICS. WE ALSO OFFER ORAL HEALTH EDUCATION AND PREVENTIVE SERVICES FOR OVER 6,600 CHILDREN AND PREGNANT WOMEN IN OUR CLINICS AND OUTREACH LOCATIONS. THE MAJORITY OF OUR PATIENTS ARE ON PUBLIC PROGRAMS (86%) OR ARE UNINSURED (5%). ABOUT HALF OF OUR PATIENTS ARE CHILDREN (UNDER THE AGE OF 21), AND 65% ARE PEOPLE OF COLOR. MANY OF OUR PATIENTS HAVE FACED BARRIERS TO ACCESSING ADEQUATE DENTAL CARE AND HAVE BEEN IMPACTED BY LONG-TERM HEALTH INEQUITIES AND DISPARITIES. WE PROVIDE CHARITY CARE TO LOW-INCOME PATIENTS NOT ELIGIBLE FOR PUBLIC PROGRAMS VIA OUR CARE GRANT PROGRAM, AND WE ALSO OFFER A SLIDING FEE SCALE OF 10-50%. IN 2021, 147 ADULTS AND CHILDREN RECEIVED CARE-GRANTS TOTALING $17,510. THROUGH THE SCHOOL-BASED SEALANT PROGRAM, 155 CHILDREN RECEIVED $15,493 IN PREVENTIVE AND RESTORATIVE CARE AND 90 UNINSURED CHILDREN RECEIVED DENTAL SERVICES TOTALING $12,725. IN 2021, 232 CHILDREN RECEIVED FREE DENTAL SERVICES WITH OUR MEDICAL/DENTAL INTEGRATION PROJECT. WE ALSO PARTNER WITH 11 MINNESOTA COLLEGES/UNIVERSITIES TO PROVIDE CLINICAL TRAINING FOR STUDENTS IN THE AREAS OF DENTAL HYGIENE, DENTAL ASSISTING, ADVANCED DENTAL THERAPY, COMMUNITY HEALTH WORKER AND NURSING.


COMMUNITY DENTAL CARE ALSO PROVIDES OUTREACH SERVICES TO PROMOTE ACCESS TO DENTAL CARE FOR CHILDREN AND PREGNANT MOTHERS AT A MEDICAL CLINIC SERVING A LOW INCOME/IMMIGRANT POPULATION IN EAST ST PAUL. THIS PROGRAM BEGAN AS A PILOT IN 2017, AND IS NOW OPERATING ON A CONSISTENT WEEKLY SCHEDULE. IN 2021, OUR COLLABORATIVE PRACTICE HYGIENIST PROVIDED CARE FOR 236 PATIENT PATIENTS OF THE FAIRVIEW ROSELAWN MEDICAL CLINIC IN CONJUNCTION WITH THEIR MEDICAL CARE VISITS. 95% OF PATIENTS SERVED ARE RACIAL/ETHNIC MINORITIES (PRIMARILY KAREN/BURMESE). MOST OF PATIENTS SERVED HAD NEVER RECEIVED DENTAL SERVICES PRIOR TO THIS VISIT. COMMUNITY DENTAL CARE PROVIDES FOLLOW-UP CARE IN OUR CLINICS TO PATIENTS AS NEEDED
