THE ORGANIZATION WAS A MULTIPLE EMPLOYER WELFARE ARRANGEMENT (MEWA) WHICH PROVIDED MEDICAL, DENTAL, VISION AND LIFE INSURANCE FOR ITS PARTICIPATING MEMBERS. THE PLAN WAS TERMINATED DECEMBER 31, 2019, ACCORDINGLY, NO NEW CLAIMS WILL BE PAID FROM THE PLAN AFTER THIS DATE. HOWEVER THE PLAN REMAINED OPEN TO PAY RUNOUT CLAIMS INCURRED PRIOR TO DECEMBER 31, 2019 DURING THE 2020 YEAR.
