THE PURPOSE OF THE TRUST IS TO FUND A SELF-FUNDED HEALTH INSURANCE PLAN FOR THE STAFFS OF UNITARIAN UNIVERSALIST CONGREGATIONS, THE UUA HEADQUARTERS AND FIELD STAFFS, AND OTHER ELIGIBLE PERSONS.

THE UNITARIAN UNIVERSALIST ASSOCIATION EMPLOYEE BENEFITS TRUST (THE "TRUST") WAS ESTABLISHED BY THE UNITARIAN UNIVERSALIST ASSOCIATION (UUA) BOARD OF DIRECTORS ON OCTOBER 22, 2006, AND BECAME EFFECTIVE ON JANUARY 1, 2007. THE PURPOSE OF THE TRUST IS TO FUND A SELF-FUNDED HEALTH INSURANCE PLAN FOR THE STAFFS OF UNITARIAN UNIVERSALIST CONGREGATIONS, THE UUA HEADQUARTERS AND FIELD STAFFS, AND OTHER ELIGIBLE PERSONS. THE TRUST ESTABLISHED THE UNITARIAN UNIVERSALIST ORGANIZATION HEALTH PLAN TO PROVIDE MEDICAL AND PRESCRIPTION DRUG BENEFITS TO ELIGIBLE EMPLOYEES AND ELIGIBLE RETIREES (AND THEIR ELIGIBLE DEPENDENTS) OF EMPLOYERS AND INDIVIDUALS WHO SUBSCRIBE TO THE PLAN AND TRUST. SUBSCRIBING EMPLOYERS INCLUDE CONGREGATIONS AND OTHER ENTITIES AFFILIATED WITH THE UUA. SUBSCRIBING INDIVIDUALS INCLUDE SELF-EMPLOYED UNITARIAN UNIVERSALIST COMMUNITY MINISTERS AND UNITARIAN UNIVERSALIST COMMUNITY MINISTERS WORKING IN A MINISTERIAL CAPACITY FOR AN EMPLOYER THAT DOES NOT OFFER A HEALTH INSURANCE PLAN.
