FULL ACCESS PROVIDED CASE MANAGEMENT SERVICES TO INDIVIDUALS WITH INTELLECTUAL AND DEVELOPMENTAL DISABILITIES. WE PROVIDED EACH INDIVIDUAL WITH A PLANNING PROCESS TO HELP IDENTIFY NOT ONLY NEEDS, BUT DREAMS AND GOALS ASSISTING THEM TO LIVE THE LIFE THEY WANT. ALONG WITH BEING ABLE TO LIVE INDEPENDENTLY OR WITH FAMILY, SOME OF THE OTHER RESULTS OF THE SERVICES PROVIDED INCLUDE HELPING INDIVIDUALS TO OBTAIN AND MAINTAIN EMPLOYMENT, BUILD ON THEIR FRIENDSHIP BASE, AND PARTICIPATE IN COMMUNITY ACTIVITIES.
