SERVICE CONCENTRATIONS INCLUDE NEIGHBORS WITH STROKE, DEVELOPMENTAL DELAY, ALZHEIMER'S AND DEMENTIA AND PARKINSON'S DISEASE. THIRTEEN FAMILIES WERE SERVED IN THE 2022 PILOT YEAR.
