DOCTORS CARE PROVIDES ACCESS TO QUALITY, AFFORDABLE HEALTH CARE AND SERVICES DESIGNED TO REDUCE BARRIERS TO HEALTH FOR LOW-INCOME PEOPLE IN NEED.

DOCTORS CARES SERVICES PROVIDE AT-RISK CHILDREN AND ADULTS ACCESS TO HIGH-QUALITY, AFFORDABLE HEALTH CARE AND SUPPORT SERVICES THAT ADDRESS IMMEDIATE NEEDS AND BUILD A FOUNDATION FOR LONG-TERM WELL-BEING. IN 2021, OVER 4,000 INDIVIDUALS HAD ACCESS TO OUR CORE PROGRAMS THAT PROVIDE A UNIQUE OPPORTUNITY FOR PATIENTS, CLIENTS, AND COMMUNITY MEMBERS TO RECEIVE WHOLE-PERSON CARE IN ONE CONVENIENT LOCATION. OVER 7,000 VISITS WERE CONDUCTED IN 2021 FOR ORAL, BEHAVIORAL, OR WOMENS HEALTH CARE AND SERVICES THAT FURTHER ALLEVIATE SOCIAL DETERMINANTS OF HEALTH. IN 2021, OVER 1,000 INDIVIDUALS RECEIVED HELP APPLYING FOR MEDICAID OR SUBSIDIZED INSURANCE PLANS THROUGH CONNECT FOR HEALTH COLORADO AND NAVIGATING CHANGES TO PLANS DUE TO LIFE EVENTS. IN ADDITION, STAFF PROVIDED OVER 1,000 RESOURCES FOR FOOD, VISION, PRESCRIPTION, BILL MANAGEMENT, AND HOUSE ASSISTANCE NEEDS WERE PROVIDED.
