SUPPORT COMMUNITY PARTNERS TO INITIATE, EXPAND, AND SUSTAIN HEALTH AND WELLNESS PROGRAMS AND SERVICES THAT ADDRESS IMPORTANT HEALTH ISSUES FACING OUR COMMUNITIES.

THROUGH A SOLE SOURCE CONTRACT WITH THE IOWA DEPARTMENT OF HEALTH AND HUMAN SERVICES, THE HUB WORKED TO ADDRESS STATEWIDE SUPPORT FOR BOTH DIABETES SELF-MANAGEMENT EDUCATION AND SUPPORT (DSMES) PROGRAMS AND THE DIABETES PREVENTION PROGRAM (DPP)AND ADDRESSED SOCIAL DETERMINANT OF HEALTH (SDOH) NEED OF COMMUNITY MEMBERS TO INCREASE ACCESIBILITY AND PARTICIPATION IN BOTH THE DSMES, DPP, AND OTHER DIABETES SUPPORT PROGRAMS. WE WORKED WITH ONBOARDING COMMUNITY-BASED ORGANIZATIONS TO THE HUB AND PROVIDED NAVIGATION SERVICES INTO PROGRAMS AND SERVICES INCLUDING MONTHLY HOME DELIVERY AOF FRESH PRODUCE TO PARTICIPANTS DEMONSTRATING FOOD AND NUTRITION INSECURITY.


CONTRACTED BY THE NATIONAL ASSOCIATION OF CHRONIC DISEASE DIRECTORS, THE HUB WORKED ON INCREASING CLINICIAN SCREENING, DETECTION, AND REFERRAL OF ADULTS WITH PRE-DIABETES TO CDC-RECOGNIZED ORGANIZATIONS BY FOCUSING ON HOW HEALTH SYSTEMS IMPLEMENT POLICY AND PRACTICE CHANGES TO IDENTIFY PATIENTS WITH PREDIABETES AND REFER THEN TO A LIFESTYLE CHANGE PROGRAM. THE HUB ALSO WORKED TO INCREASE ENROLLMENT AND RETENTION RATES FOR PARTICIPANTS IN THIS LIFESTYLE CHANGE PROGRAM BY DEVELOPING AND/OR ADAPTING TOOLS, MATERIALS, BEST PRACTICES, AND ADVANCED SKILLS TRAINING FOR COMMUNITY COACHES TO SUPPORT AND RETAIN THEIR PARTICIPANTS.


THE HUB WORKED WITH IOWA STATE UNIVERSITY AS THE CO-PI ON TWO CDC GRANT PROJEC ABD SUPPORTING CAPACITYTS TO EXPAND ARTHRITIS-APPROPRIATE EVIDENCE-BASED PROGRAMS. GRANT PROJECT ACTIVITIES INCLUDED REDUCING RISKS OF FALLS IN OLDER ADULTS AS A PUBLIC HEALTH PRIORITY DUE TO THE HIGH PREVALENCE AND SIGNIFICAN IMPACT ON HEALTH CARE COSTS. WE HELPED WITH COMMUNITY FALL SCREENING EVENTS AND CONNECTING INDIVIDUALS WITH RISK FOR FALLS TO COMMUNITY PROGRAMS. ACTIVITES ALSO INCLUDED BUILDING AND SUPPORTING CAPACITY FOR THE BROAD DISSEMINATION OF ARTHRITIS-APPROPRIATE, EVIDENCE-BASED INTERVENTIONS (AAEBI'S) TO REDUCE THE BURDEN OF ARTHRITIS IN IOWA.
