CONCORDIA HOSPITAL FOUNDATION WAS FORMED TO ASSIST IN RAISING ADDITIONAL CAPITAL TO CONSTRUCT, EQUIP, AND OPERATE A REPLACEMENT HOSPITAL FACILITY FOR CONCORDIA PARISH HOSPITAL SERVICE DISTRICT NUMBER ONE OF THE PARISH OF CONCORDIA IN ORDER TO PROVIDE THE MEDICALLY UNDERSERVED AND PARTICULARLY THE UNINSURED, UNDERINSURED AND INDIGENT CITIZENS OF THE PARISH WITH ACCESS TO QUALITY, AFFORDABLE AND INDISPENSABLE EMERGENCY AND RELATED HEALTHCARE SERVICES.ON MARCH 18, 2019, THE CONCORDIA HOSPITAL FOUNDATION ENTERED INTO A DEBT TRANSACTION TO ACCESS ADDITIONAL FUNDS THROUGH THE NEW MARKET TAX CREDIT (NMTC) PROGRAM. CONTINUED ON SCHEDULE O.THESE FUNDS ARE BEING USED TOWARD THE CONSTRUCTION OF A NEW HOSPITAL BUILDING FOR CONCORDIA PARISH HOSPITAL SERVICE DISTRICT NUMBER ONE OF THE PARISH OF CONCORDIA, STATE OF LOUISIANA DBA RIVERLAND MEDICAL CENTER LOCATED IN FERRIDAY, LOUISIANA. THE NMTC PROGRAM PERMITS TAXPAYERS TO CLAIM FEDERAL TAX CREDITS FOR MAKING QUALIFIED EQUITY INVESTMENTS (QEI) IN A DESIGNATED COMMUNITY DEVELOPMENT ENTITY (CDE). THE CDE MUST USE SUBSTANTIALLY ALL OF THE PROCEEDS TO MAKE QUALIFIED LOW-INCOME COMMUNITY INVESTMENTS (QLICI).
