THE MODEL INVOLVES AN INTERDISCIPLINARY APPROACH TO THE CARE OF THE PATIENT. WHEN A PATIENT COMES IN FOR A COMPREHENSIVE VISIT THEY ARE SEEN BY A CLINICAL TEAM CONSISTING OF A HEMATOLOGIST, NURSE PRACTITIONER, (OR PHYSICIAN ASSISTANT) A NURSE, A PHYSICAL THERAPIST (IF NECESSARY) AND A SOCIAL WORKER.
