THE MISSION OF ADULT CARE MANAGEMENT, INC. (ACMI) IS TO ENABLE THE ELDERLY, THE CHRONICALLY ILL AND PERSONS WITH DISABILITIES TO MAINTAIN THEIR LIFESTYLE AND DIGNITY THROUGH CARE MANAGEMENT. ACMI PROVIDES FUNCTIONAL EVALUATIONS OF INDIVIDUALS' SELF-CARE STRENGTHS AND WEAKNESSES, ARRANGES FOR THE MOST APPROPRIATE LONG-TERM SUPPORTS AND CARE SERVICES, AND PROVIDES ONGOING MONITORING OF SERVICES AND NEEDS. SERVICES AND SUPPORTS MAY INCLUDE MEDICAID WAIVER SERVICES, GENERAL MEDICAID BENEFITS AND OTHER COMMUNITY-BASED RESOURCES. THE GOAL OF ACMI'S CARE MANAGEMENT SERVICE IS TO PREVENT INSTITUTIONAL STAYS AND SUPPORT THE INDIVIDUAL'S SUCCESS IN THE COMMUNITY.

OPTIONS FOR LONG TERM CARE PROVIDES CASE MANAGEMENT AND CARE COORDINATION SERVICES TO APPROXIMATELY 3,167 CHILDREN AND DISABLED ADULTS UNDER THE MEDICAID WAIVERS IN COLORADO. HCBS CHILDREN'S PROVIDES CASE MANAGEMENT AND CARE COORDINATION SERVICES TO 56 CHILDREN UNDER THE MEDICAID WAIVERS IN COLORADO. HOME CARE ALLOWANCE (HCA) PROVIDES STATE-ONLY FUNDED CASE MANAGEMENT AND CARE COORDINATION SERVICES TO 51 CHILDREN AND ADULTS UNDER THE COLORADO DEPARTMENT OF HUMAN SERVICES (CDHS).
