THE ACME ENGINEERING COMPREHENSIVE HEALTH CARE TRUST IS A VOLUNTARY EMPLOYEE'S BENEFICIARY ASSOCIATION UNDER I.R.C. SECTION 501(C)(9). THE PLAN PROVIDES HEALTH BENEFITS (DENTAL, MEDICAL, HOSPITAL, SURGICAL, TRANSPLANT, AND MAJOR MEDICAL) TO ELIGIBLE PARTICIPANTS AND COVERED DEPENDENTS. THE PLAN ALSO PROVIDES CONTINUATION OF CERTAIN BENEFITS UPON TERMINATION OF EMPLOYMENT THROUGH THE CONSOLIDATED OMNIBUS BUDGET RECONCILIATION ACT (COBRA).
