NUTRITION SERVICES SENIOR CONNECTIONS PRODUCED NUTRITIONALLY-BALANCED, FRESHLY PREPARED MEALS AND DELIVERED THESE MEALS ON WHEELS PROGRAM MEALS WERE DISTRIBUTED TO SENIORS IN THEIR HOMES, AT ADULT DAY CARE SITES, SENIOR CENTERS AND A CHARTER SCHOOL DELIVERY DESTINATIONS INCLUDED DEKALB, FULTON, ROCKDALE, AND BIBB COUNTIES SENIOR CONNECTIONS OFFERS A VARIETY OF THERAPEUTIC MENUS INCLUDING DIABETIC, RENAL, MECHANICAL SOFT, LOW SODIUM, NO SEAFOOD AND VEGETARIAN AND THE MEALS ARE AVAILABLE FOR HOME DELIVERY AND AT SENIOR CENTERS MEALS ON WHEELS ARE DELIVERED IN DEKALB COUNTY VIA VOLUNTEERS DIRECTLY TO THE SENIORS' HOME SIX DAYS A WEEK
SENIOR CENTERS SENIOR CONNECTIONS SERVED OVER 71,000 SENIORS IN DEKALB COUNTY AT FOUR SENIOR CENTERS IN FY 2018 SENIORS AT CENTERS RECEIVE A MEAL AND PARTICIPATE IN PROGRAMS INCLUDING EXERCISE PROGRAMS AND VARIOUS OTHER ARTS AND CRAFTS AND EDUCATIONAL PROGRAMS THE CENTERS ARE LOCATED IN NORTH DEKALB, SOUTH DEKALB, DEKALB-ATLANTA WHICH IS IN THE CITY OF ATLANTA IN DEKALB COUNTY AND THE LITHONIA CENTER LOCATED IN LITHONIA
HOME REPAIR SENIOR CONNECTIONS REPAIRED HOMES IN FY 2017 AND EXPENSED THE AMOUNTS RELATING TO THE REPAIRS. HOME REPAIR PROJECTS WERE MAINLY IN THE CITY OF ATLANTA IN FULTON COUNTY WITH A FEW HOMES BEING REPAIRED IN DEKALB COUNTY BUT MOST FUNDING WAS ALLOCATED FOR CITY OF ATLANTA SENIOR HOMEOWNERS REPAIRS WERE EMERGENCY REPAIRS INCLUDING ROOFS, PLUMBING, AND ELECTRICAL.
IN-HOME SENIOR CONNECTIONS PROVIDES UNSKILLED CARE INCLUDING PERSONAL CARE, HOMEMAKER AND RESPITE CARE TO SENIORS IN DEKALB AND FULTON COUNTIES IN FY 2017 IN-HOME CARE WAS PROVIDED TO OVER 1,000 SENIORS IN DEKALB AND FULTON COUNTIES SENIOR CONNNECTIONS CONTINUES TO PARTICIPATE IN THE CARE TRANSITION PROGRAM.
