THE PROGRAM PROVIDES ASSISTANCE FOR COMMUNITIES TO COMPLETE A HEALTH SYSTEM PROFILE AND COMMUNITY-WIDE STRATEGIC PLAN FOR IMPROVING THE EFFECTIVENESS AND EFFICIENCY OF THE COMMUNITY'S HEALTHCARE DELIVERY SYSTEM.ALL PROGRAMS CONTINUE WITH TREMENDOUS SUCCESS.
