HOSPICE FOCUSES ON THE PHYSICAL, EMOTIONAL AND SPIRITUAL NEEDS OF PATIENTS AND THEIR FAMILIES DURING ILLNESS, DEATH AND BEREAVEMENT. THROUGH END-OF-LIFE HOSPICE CARE, TRANSITIONAL PROGRAMS AND SERVICES, AND COMMUNITY OUTREACH, LCFH & LCC STRIVES TO HELP ITS COMMUNITY MEMBERS LIVE THEIR LIVES WELL, HOWEVER SHORT OR LONG THEIR LIVES MAY BE.
