Part 3
MOD(MENTOR ON DISCHARGE) PROGRAM, AS OF JULY 1 2018, WE HAVE EFFECTIVELY SERVED OVER 90 PATIENTS. OBJECTIVE IS TO REDUCE RE-HOSPITALIZATIONS. OF 90 SERVED, MORE THAN 70% WERE NOT RE-HOSPITALIZED. BOTH MOD AND MENTOR ON DISCHARGE ARE REGISTERED SERVICEMARKS OF NAMI ACS.
IOOV (IN OUR OWN VOICE) TWO PEOPLE TELL THEIR STORY OF MENTAL HEALTH RECOVERY. IOOV PRESENTATIONS WERE MADE IN 3 PSYCHIATRIC HOSPITALS TO OVER 4,000 PATIENTS. WITH KNOWLEDGE THAT RECOVERY IS POSSIBLE PATIENTS ARE MORE WILLING TO WORK ON TREATMENT PLANS WITH CLINICIANS.
P2P (PEER TO PEER) NAMI PEER-TO-PEER IS AN EIGHT-SESSION EDUCATIONAL PROGRAM FOR ADULTS WITH MENTAL HEALTH CONDITIONS WHO ARE LOOKING TO BETTER UNDERSTAND THEMSELVES AND THEIR RECOVERY.
