SERVICE COORDINATORS WORK TO OBTAIN SERVICES FROM LOCAL RESOURCES AND LINK THOSE SERVICES TO ACHIEVEMAXIMUM INDEPENDENCE FOR OUR PARTICIPANTS.SERVICE COORDINATORS HELP PARTICIPANTS IN SETTING UP A PLAN FOR CARE. THEY ALSO ASSIST THE PARTICIPANT IN CHOOSING AN AGENCY TO PROVIDE THE IN-HOME CARE SERVICES.IN-HOME CARE SERVICES SUPPORT ADULTS WITH PHYSICAL DISABILITIES WHO NEED HELP TO PERFORM DAILY LIVING TASKS.
