CARE MANAGEMENT - OFFERS COMMUNITY AND HOME CARE OPTIONS TO THE FRAIL ELDERLY. IT IS DESIGNED TO ASSIST OLDER ADULTS WHO WANT TO REMAIN AT HOME BUT REQUIRE HOME AND COMMUNITY-BASED SERVICES TO DO SO. 1,990 SERVED.
MI HEALTH LINK - PROVIDES CARE COORDINATION AND SUPPORT SERVICES FOR MEMBERS OF INTEGRATED CARE ORGANIZATIONS UNDER THE STATE OF MICHIGAN DUAL ELIGIBLE PROGRAM. 2,326 SERVED.
SOCIAL SERVICES - PROVIDES A VARIETY OF SERVICES TO SENIOR CITIZENS IN DETROIT, THE FIVE GROSSE POINTES, HAMTRAMCK, HARPER WOODS, AND HIGHLAND PARK. 27,934 SERVED.
