NUTRITION - PROVIDES MEALS THROUGH AN EXTENSIVE HOME-DELIVERED MEALS ON WHEELS NETWORK AND AT COMMUNITY SITES (ALSO KNOWN AS CONGREGATE) MONDAY THROUGH FRIDAY (BARRING HOLIDAY OR EMERGENCY CLOSINGS), WITH WEEKEND MEAL OPTIONS AVAILABLE, IN A NINE-COUNTY REGION OF NORTHEAST INDIANA.


AREA AGENCY ON AGING - AS THE AREA III AREA AGENCY ON AGING, THE ORGANIZATION IS THE PRIMARY RESOURCE FOR AGING AND DISABLED POPULATIONS IN EITHER PROVISION OF SERVICES OR FUNDING SERVICES.


CASE MANAGEMENT - A FORMAL SYSTEM OF ASSESSMENT, CARE PLANNING, AND EVALUATION OF SERVICES TO ASSIST QUALIFIED CLIENTS IN REALIZING AND MAINTAINING THEIR INDEPENDENCE. ADDITIONALLY, THE PROGRAM DIVERTS INDIVIDUALS FROM NURSING HOMES, ALLOWING THEM TO REMAIN SAFELY IN THEIR HOMES. ALSO, PRE-ADMISSION SCREENINGS ARE PERFORMED TO DETERMINE AN INDIVIDUAL'S ELIGIBILITY FOR ADMISSION TO, OR CONTINUED RESIDENCE IN, A NURSING FACILITY IN OUR NINE-COUNTY REGION.
