REACHING OUT TO ADULTS AND CHILDREN IN NORTHEAST OHIO TO END HOMELESSNESS, PREVENT SUICIDE, RESOLVE BEHAVIORAL HEALTH CRISES AND OVERCOME TRAUMA

PERMANENT HOUSING WITH ON-SITE SUPPORTIVE SERVICES: PROVIDED SPECIALIZED SUPPORTIVE AND PSYCHIATRIC SERVICES FOR CHRONICALLY HOMELESS PERSONS WHO HAVE A SEVERE MENTAL DISABILITY AND THE NEED TO LEARN DAILY LIVING SKILLS. DURING FISCAL YEAR 2021, 7,214 HOMELESS ADULTS WERE TREATED FOR ONE OR MORE OF THE FOLLOWING: SEVERE MENTAL DISABILITY, PHYSICAL ILLNESS, PHYSICAL DISABILITY, DRUG ADDICTION, ALCOHOL ADDICTION, ETC.


CRISIS AND TRAUMA INTERVENTION: CHILDREN AND ADULTS WHOSE SAFETY AND HEALTH ARE THREATENED BY MENTAL ILLNESS OR OVERWHELMING STRESSORS CALL OUR 24/7 SUICIDE HOTLINE. OUR MOBILE CRISIS TEAM HELPS THEM ACHIEVE SAFE RESOLUTION OF THEIR CRISIS SITUATIONS. THESE SERVICES FORMED THE EFFECTIVENESS OF MOBILE CRISIS OUTREACH. DURING FISCAL YEAR 2021, 11,925 CHILDREN AND ADULTS TOOK PART IN CRISIS AND TRAUMA INTERVENTION SERVICES.


OUTREACH AND COMMUNITY SUPPORT: PROVIDES DIAGNOSTIC ASSESSMENT, COMMUNITY SUPPORT PROGRAM, PSYCHIATRIC AND NURSING SERVICES AND OUTREACH SUPPORTIVE SERVICES FOR HOMELESS ADULTS WITH SEVERE MENTAL DISABILITY. PROVIDES EVIDENCE-BASED MENTAL HEALTH AND SUPPORTIVE SERVICES IN ORDER TO HELP THOSE WITH SEVERE MENTAL ILLNESS ACHIEVE COMPETITIVE EMPLOYMENT AND RECOVERY. THIS PROGRAM SERVED 759 INDIVIDUALS DURING FISCAL YEAR 2021.
