PROVIDING THE HIGHEST QUALITY OF MEDICAL CARE TO THEIR PATIENTS IN SOUTHWESTERN CONNECTICUT; SPECIALIZING IN PRIMARY CARE FOR PEOPLE LIVING WITH HIV, THE LGBTQ COMMUNITY, AS WELL AS HIV PREVENTION (PREP & PEP), HEPATITIS C, AND STI TESTING AND TREATMENT. THE ORGANIZATION'S PROGRAMS INCLUDE MEDICAL AND PHARMACY SERVICES.

CLINICAL AND HIV OUTREACH TESTING INITIATIVE SERVING 2,115 PATIENTS.
PHARMACY FILLING 20,954 PRESCRIPTIONS.
MEDICAL SERVICES: THE ORGANIZATION, THROUGH ITS CIRCLE CENTER CARE CLINIC, PROVIDES HIGH-QUALITY, SENSITIVE AND COMPREHENSIVE PRIMARY CARE TARGETED TO MEET THE HEALTH NEEDS OF ALL OF THEIR PATIENTS. THEIR PROVIDERS SPECIALIZE IN WORKING WITH PEOPLE LIVING WITH HIV AND MEMBERS OF THE LGBTQ COMMUNITY. THE CARE INCLUDES A RELATIONSHIP WITH A PRIMARY MEDICAL PROVIDER ACCOMPANIED BY A TEAM OF CLINICAL SUPPORT STAFF, ANNUAL WELLNESS VISITS INCLUDING PREVENTATIVE SCREENINGS, GENERAL MEDICAL SERVICES FOR COMMON AILMENTS (FLU, FEVERS, SEXUALLY TRANSMITTED INFECTIONS, ETC.), MANAGEMENT OF CHRONIC HEALTH CONDITIONS SUCH AS HIV/AIDS, DIABETES, ASTHMA, HIGH BLOOD PRESSURE, DEPRESSION, AMONG OTHERS, BILINGUAL STAFF FLUENT IN BOTH ENGLISH AND SPANISH, AND ON-SITE BLOOD DRAWS.
PHARMACY SERVICES: THROUGH CIRCLE CARE CENTER PHARMACY THE ORGANIZATION OFFERS FULL-SERVICE SPECIALTY PHARMACY, WITH MONTHLY PRESCRIPTION AUTO-FILL AVAILABLE. CONVENIENT PRESCRIPTION PICKUP WITH MINIMAL WAIT TIME, MOST INSURANCE PLANS ACCEPTED, INCLUDING MEDICAID, MEDICARE AND CADAP. PRIVATE AND PERSONALIZED MEDICATION CONSULTATION IS ALSO AVAILABLE.
