THESE SERVICES ALLOW THEM TO REMAIN IN THEIR HOMES, PROMOTE THEIR INDEPENDENCE, AND DELAY OR PREVENT TRANSITION TO A NURSING HOME OR ASSISTED LIVING ENVIRONMENT. THE LHCPA PROVIDED A TOTAL OF 11,336 VISITS TO 142 INDIVIDUALS.
