WE TREAT IMMEDIATE NEEDS AND BRIDGE CARE TO A PERMANENT MEDICAL HOME. BEHAVIROAL HEALTH-OFFER MENTAL HEALTH COUNSELING AND PSYCHIATRIC MEDICATION MANAGEMENT DEPENDING UPON THE NEEDS AND PREFERENCES OF THE PATIENT AND MANAGED COLLABORATIVELY BETWEEN PERSCRIBERS AND COUNSELORS. SPECIALTY CARE-SERVICES INCLUDE REPRODUCTIVE HEALTH, GENDER HEALTH PHYSICAL THERAPY, AND CHIROPRACTIC CARE. REFERRALS ARE PROVIDED TO COMMUNITY PARTNERS WHEN ADDITIONAL SPECIALTY CARE IS NEEDED.
