THE ORGANIZATION FACILITATES PERMANENT SUPPORTIVE HOUSING FOR HOMELESS TRANSITION AGEYOUTH, AGES 18-24, WHOM HAVE AGED OUT OF THE FOSTER CARE SYSTEM AND WHO ARE WILLINGAND ABLE TO WORK TOWARDS SELF-SUFFICIENCY. THE SUPPORTIVE HOUSING SERVICES INCLUDE,BUT NOT LIMITED TO COUNSELING/CASE MANAGEMENT; EMPLOYMENT PREPARATION AND REFERRAL;ALCOHOL, TOBACCO, AND DRUG ABUSE PREVENTION, INTERVENTION AND EDUCATION; MENTALHEALTH REFERRALS; PUBLIC BENEFITS; REFERRAL TO OUTSIDE SERVICES; INDEPENDENT LIVINGSKILLS; COMMUNITY INVOLVEMENT; AND RECREATION.
