TO PROVIDE SAFE, AFFORDABLE PERMANENT HOUSING TO FORMERLY HOMELESS INDIVIDUALS AND FAMILIES AND TO HELP THEM TO ACHIEVE INCREASED LEVELS OF SELF-RELIANCE AND STABILITY. COMPREHENSIVE SUPPORT SERVICES ARE PROVIDED TO RESIDENTS AS AN INTEGRAL COMPONENT OF THE HOUSING.

Part 3 - Line 4a

PROVIDE AFFORDABLE HOUSING TO SINGLE ADULTS AND FAMILIES THAT WERE FORMERLY HOMELESS. PROGRAM PARTICIPANTS ARE PROVIDED WITH SUPPORT SERVICES TO HELP THEM GAIN AND MAINTAIN HOUSING, INCOME AND EMOTIONAL STABILITY. THE ORGANIZATION ASSISTS FORMERLY HOMELESS HOUSEHOLDS TO ACHIEVE INCREASED LEVELS OF SELF-RELIANCE THROUGH THE SUCCESSFUL TENANCY AMOUNG RESIDENTS PROGRAM. THIS PROGRAM IS A HOMELESSNESS PREVENTION PROGRAM THROUGH WHICH PARTICIPANTS ARE PROVIDED ASSISTANCE TO LEARN THE SKILLS NECESSARY FOR SUCCESSFUL HOUSEHOLD MANAGEMENT, COMMUNITY INVOLVEMENT, VOLUNTEERISM, SELF ADVOCACY, COPING SKILLS, INTERPERSONAL SKILLS, CRISIS RESOLUTION, AND GOAL SETTING. SOME PARTICIPANTS REQUIRE ASSISTANCE WITH COOKING AND NUTRITION SKILLS, BANKING, MEDICATION COMPLIANCE, AND TRANSPORTATION. ACTION PLANS ARE CRAFTED TO PROVIDE AN INDIVIDUALIZED RESPONSE TO HOUSEHOLD NEEDS AND DESIGNED TO DECREASE IN FREQUENCY AND INTENSITY AS STABILITY IN HOUSING AND COMMUNITY SUPPORT IS ACHIEVED.
