THE PARTNERSHIP FOR PUBLIC HEALTH WAS ORGANIZED TO IMPROVE THE HEALTH & WELL-BEING OF THE REGION THROUGH INTERORGANIZATIONAL COLLABORATION, AND COMMUNITY AND PUBLIC HEALTH IMPROVEMENT ACTIVITIES.

THE ORGANIZATION'S MAJOR ACTIVITIES THIS YEAR INCLUDE 1) SERVICELINK - SERVICES INCLUDE COMPREHENSIVE NEEDS ASSESSMENT, EVENING AND WEEKEND OUTREACH, FOLLOW UP, LONG-TERM CARE COUNSELING, INFORMATION & REFERRAL, MEDICARE COUNSELING SERVICES, MEDICARE PRESCRIPTION PLAN INFORMATION & ASSISTANCE, CAREGIVER SERVICES, VETERAN'S DIRECTED HOME & COMMUNITY BASED SERVICES AND SUPPORT ASSISTIVE TECHNOLOGY LOAN AND DEMONSTRATION PROGRAM, & PUBLIC EDUCATION, TOTAL CONTACTS THIS YEAR WERE APPROXIMATELY 8,000.


NH'S DELIVERY SYSTEM REFORM INCENTIVE PAYMENT PROGRAM (DSRIP) (1115 MEDICAID WAIVER) - WITH A PRIMARY FOCUS ON MEDICAID CLIENTS DESIGNED TO 1) INCREASE THE CAPACITY OF MENTAL HEALTH AND SUBSTANCE ABUSE DISORDER TREATMENT 2)PROMOTE INTEGRATION OF PHYSICAL AND BEHAVIORAL HEALTH SERVICES AND 3)IMPROVE CARE TRANSITIONS BY FACILITATING SMOOTH CONNECTION TO PATIENT SERVICES. TOTAL CONTACTS WERE APPROXIMATELY 2,000 THIS YEAR.


1) PARTNERS IN COMMUNITY WELLNESS - WORK WITH LOCAL COALITIONS, HEALTH CARE PROVIDERS, AND OTHERS ACROSS THE CONTINUUM OF CARE TO REDUCE THE IMPACT OF SUBSTANCE MISUSE, SUICIDE, AND OTHER BEHAVIORAL HEALTH CONCERNS. 2) PUBLIC HEALTH EMERGENCY PREPAREDNESS - CONTINUE TO REVISE/ENHANCE AND TEST A PUBLIC HEALTH EMERGENCY PLAN & COMMUNITY EMERGENCY RESPONSE TEAM (CERT, MEDICAL RESERVE CORPS.) 3) HEALTHY EATING/ACTIVE LIVING - PROVIDE TECHNICAL ASSISTANCE, SUPPORT & PLANNING ACTIVITIES TO INCREASE ACCESS TO HEALTHY FOODS & PHYSICAL ACTIVITY. 4) OTHER PUBLIC HEALTH ACTIVITIES - COMMUNITY HEALTH IMPROVEMENT PLAN, IMMIGRATION INTEGRATION, FLU VACCINATIONS, AND ORAL HEALTH EDUCATION. PROVIDED APPROXIMATELY 800 FLU VACCINATIONS TO AREA SCHOOL CHILDREN AND SUPPORTED THE DISTRIBUTION OF MORE THAN 30,000 COVID VACCINATIONS.
