HEALTH HOME CARE MANAGEMENT SERVICES IS A GROUP OF HEALTH CARE AND SERVICE PROVIDERS WORKING TOGETHER TO MAKE SURE A CONSUMERS GETS THE CARE AND SERVICES THEY NEED TO STAY HEALTHY. ONCE THEY ARE ENROLLED IN A HEALTH HOME, THEY WILL HAVE A CARE MANAGER THAT WORKS WITH THEM TO DEVELOP A CARE PLAN. A CARE PLAN MAPS OUT THE SERVICES A CONSUMER NEEDS TO PUT THEM ON THE ROAD TO BETTER HEALTH
THE OBJECTIVE OF THE HOUSING PROGRAM IS TO ASSIST INDIVIDUALS IN LOCATING AND SECURING HOUSING OF THEIR CHOICEAND IN ACCESSING THE SUPPORTS NECESSARY TO LIVE SUCCESSFULLY IN THE COMMUNITY. RENTAL ASSISTANCE IS PROVIDED TO RESIDENTS OF SUPPORTIVE HOUSING PROGRAMS THROUGH THE MEANS OF A VOLUNTARY AGENCY-ADMINISTERED RENT STIPEND MECHANISM
OUTREACH PROGRAMS/SERVICES ARE INTENDED TO ENGAGE AND/OR ASSESS INDIVIDUALS POTENTIALLY IN NEED OF MENTAL HEALTH SERVICES. OUTREACH PROGRAMS/SERVICES ARE NOT CRISIS SERVICES. THESE SERVICES CAN BE PROVIDED AT FORENSIC SITES, A CONSUMER'S HOME, OTHER RESIDENTIAL SETTINGS, INCLUDING HOMELESS SHELTERS, AND THE STREETS.
