Part 3 - Line 1

TO PROVIDE PERSON-CENTERED SERVICES AND SUPPORTS TO CHILDREN AND ADULTS WHO HAVE DEVELOPMENTAL DISABILITIES. THE GOAL OF THESE SERVICES AND SUPPORTS IS TO PROMOTE INDEPENDENCE, COMMUNITY INVOLVEMENT, AND QUALITY OF LIFE.

Part 3 - Line 4a

RESIDENTIAL SERVICES: PROVIDE A HOME-LIKE ENVIRONMENT TO 81 ADULTS WITH SEVERE DISABLING CONDITIONS IN NINE GROUP HOMES. OVERSEE MEDICAL CARE AND ACTIVITIES OF DAILY LIVING WHILE PROMOTING INDEPENDENCE AND COMMUNITY INCLUSION.


ADULT DAY SERVICES: TEACHING SKILLS WHICH ENABLE ADULTS TO BE MORE INDEPENDENT IN THEIR USE OF COMMUNITY RESOURCES. PARTICIPANTS DEVELOP TRAVEL AND MONEY-HANDLING SKILLS, AND VOLUNTEER AT VARIOUS COMMUNITY SITES SUCH AS PARKS AND SENIOR CENTERS. SPEECH, PHYSICAL AND OCCUPATIONAL THERAPIES ARE PROVIDED AS NEEDED.


CHILDREN'S CENTER SERVICES: PROVIDES EDUCATIONAL AND TREATMENT SERVICES TO CHILDREN FROM EARLY CHILDHOOD THROUGH AGE 21. ENABLES CHILDREN AGED 3-5 WITH SEVERE DISABILITIES TO TRANSITION TO PUBLIC SCHOOL SYSTEMS.
