ALL SERVICES THAT ARE PROVIDED INCLUDE A THOROUGH ASSESSMENT OF THE INDIVIDUAL AND THEIR NEEDS, A PHYSICIAN'S STATEMENT AND APPROPRIATE RELEASES AND EMERGENCY MEDICAL DOCUMENTS.
