CONNECTED LIVING - ASSIST SENIORS AND DISABLED INDIVIDUALS TO INDEPENDENCE BY PROVIDING MEALS, TRANSPORTATION, AND RECREATIONAL ACTIVITIES TO ENHANCE THEIR DIGNITY AND ENCOURAGE CONTINUED INVOLVEMENT IN THE COMMUNITY, PROVIDE SERVICES THAT PROMOTE INDEPENDENT LIVING TO SENIORS AND DISABLED ADULTS.
