PARTICIPATING IN THE CENTER FOR MEDICARE AND MEDICAID SERVICES ("CMS") ACO PROGRAMS.IMPLEMENTING HEALTHCARE PROGRAMS DESIGNED TO PROMOTE QUALITY IMPROVEMENTS AND COST SAVINGS AND THEREBY LESSENING THE GOVERNMENT'S BURDEN ASSOCIATED WITH PROVIDING MEDICARE BENEFITS.ESTABLISHING AN INTEGRATED HEALTHCARE DELIVERY SYSTEM THAT IMPROVES THE QUALITY OF MEDICAL, SURGICAL, BEHAVIORAL AND OTHER RELATED HEALTH CARE SERVICES PROVIDED TO PATIENTS THROUGH FOSTERING SUCH INITIATIVES AS CLINICAL INTEGRATION AMONG PRIMARY CARE PHYSICIANS AND SPECIALISTS AND AMONG COMMUNITY AND ACADEMIC PHYSICIANS AND HOSPITALS, IMPLEMENTING COORDINATED ELECTRONIC HEALTH RECORD SYSTEMS, DEVELOPING BEST-PRACTICE-BASED PROTOCOLS, AND PROVIDING WELLNESS CARE AND OTHER TECHNIQUES AND SYSTEMS.IMPROVING THE PROVISION OF MEDICAL AND HEALTH CARE TO PATIENTS THROUGH THE CREATION OF A DATA BASE OF MEDICAL RECORDS, PUBLIC HEALTH INFORMATION AND POPULATION DEMOGRAPHICS.DEVELOPING FINANCIAL INCENTIVES THROUGH PAYER CONTRACTING TO IMPROVE THE QUALITY, EFFICIENCY AND EFFECTIVENESS OF CARE AND TO ENHANCE HEALTH CARE OUTCOMES FOR THE POPULATIONS SERVED BY THE PHYSICIANS AND HOSPITALS ASSOCIATED WITH THE CORPORATION.
