THE MISSION OF SENIOR LIVING PARTNERS OF VIRGINIA, LLC (SLPV) IS TO PROVIDE LOW-COST HEALTH CARE SERVICES IN PATIENTS' HOMES TO PERSONS WHO SUFFER INFIRMITIES DUE TO ADVANCED AGE, DISABILITY OR A MEDICAL CONDITION WHICH NECESSITATES ASSISTANCE.SLPV WILL PROVIDES SERVICES TO ELDERLY AND DISABLED MEMBERS OF THE GENERAL PUBLIC IN THE FORM OF LOW-COST SKILLED HOME HEALTH SERVICES, HOME CARE ASSISTANCE AND HOSPICE. SLPV HOME HEALTH PROVIDES PROFESSIONAL NURSING SERVICES AND OTHER THERAPEUTIC SERVICES TO PATIENTS IN THEIR HOMES. SLPV HOME HEALTH SERVICES ARE PROVIDED BASED ON A COURSE OF TREATMENT PRESCRIBED IN WRITING BY THE RECIPIENTS' PHYSICIAN. THE PROGRESS OF THE TREATMENT IS PERIODICALLY REVIEWED BY THE RECIPIENTS' PHYSICIAN. MOST OF SLPV'S HOME HEALTH RECIPIENTS ARE ELDERLY PEOPLE WHO ARE CONFINED BECAUSE OF THEIR INFIRMITIES OR DUE TO THE NEED FOR REHABILITATION RESULTING FROM INJURY OR SURGERY.
