("APPLICANT") CARRIES OUT CHARITABLE PURPOSES BY PROVIDING COMPREHENSIVE ANDCOMPASSIONATE RE-ENTRY SUPPORT SERVICES IN THE GREATER BAY AREA TO JUSTICE-INVOLVED INDIVIDUALS LIVING WITH SIGNIFICANT MENTAL HEALTH CHALLENGES. THROUGH ITS PROGRAMS, APPLICANT OFFERS SUPERVISED AND SUPPORTIVE HOUSING, ONSITE BEHAVIORAL HEALTH SERVICES, AND INTEGRATED WELLNESS PROGRAMMING WITHIN A WARM AND WELCOMING ENVIRONMENT. APPLICANT STRIVES TO PROVIDE ALL THE SERVICES AN INDIVIDUAL MAY REQUIRE TO SAFELY TRANSITION BACK TO THE COMMUNITY FROM MORE RESTRICTIVE SETTINGS, INCLUDING RESIDENTIAL TREATMENT, HOSPITALIZATION, AND/OR INCARCERATION.
