TRANSPORTATION PROGRAMS: THE ORGANIZATION PROVIDES TRANSPORTATION ASSISTANCE TO INDIVIDUALS WITH LIMITED MOBILITY. ASSISTANCE IS PROVIDED THROUGH REIMBURSEMENT FOR MILEAGE.


SELF DIRECTED CARE PROGRAM (FORMERLY KNOWN AS CONSUMER DIRECTED CARE): THE ORGANIZATION ASSISTS MEDICAID ELIGIBLE INDIVIDUALS WITH SIGNIFICANT NEED FOR IN HOME CARE TO REMAIN IN THEIR HOMES BY ACTING AS THE EMPLOYER FOR IN HOME CARE ASSISTANTS OF THE INDIVIDUALS' CHOOSING.
