THE HOUSE CALL PROJECT, INC. ("THCP") WORKS TO EDUCATE ON WAYS TO REDUCE COSTLY HOSPITALIZATIONS, IMPROVE THE QUALITY OF RESIDENTIAL CARE AND PROVIDE QUALITY PALLIATIVE AND END-OF-LIFE CARE. HCCI ALSO DEVELOPS EDUCATIONAL PROGRAMS INFORMING INTERESTED PARTIES ON HOW TO DEVELOP HOME CARE PROGRAMS AND PRACTICES AND HOW TO COLLABORATE WITH HOME CARE MEDICAL PROVIDERS AND CAREGIVERS TO PROVIDE APPROPRIATE AND COMPASSIONATE CARE FOR MEDICALLY COMPLEX PATIENTS, PRIMARILY HOMEBOUND ELDERLY PATIENTS. THE COMPANY ALSO INTENDS TO PROVIDE EDUCATIONAL PROGRAMS TO PROVIDER ORGANIZATIONS, SUCH AS CLINICAL INTEGRATION ORGANIZATIONS AND ACCOUNTABLE CARE ORGANIZATIONS, TO ADDRESS BEST PRACTICES AND COMPASSIONATE STRATEGIES FOR HOME CENTERED CARE EFFORTS.
