TO ESTABLISH A PROGRAM OF ACCREDITED CENTERS WITH EXPERTISE IN PULMONARY HYPERTENSION TO IMPROVE OVERALL QUALITY OF CARE AND LONG-TERM OUTCOMES OF PATIENTS WITH THIS LIFE THREATENING DISEASE.

PULMONARY HYPERTENSION CARE CENTERS (PHCC ACCREDITATION): THE PULMONARY HYPERTENSION ASSOCIATION (PHA) ESTABLISHED ITS PH CARE CENTERS (PHCC) PROGRAM TO ACCREDIT CENTERS NATIONWIDE WITH SPECIAL EXPERTISE IN TREATING PULMONARY HYPERTENSION (PH), PARTICULARLY PULMONARY ARTERIAL HYPERTENSION (PAH), TO RAISE THE OVERALL QUALITY OF CARE AND OUTCOMES IN PATIENTS WITH THIS INCURABLE, PROGRESSIVE, LIFE-THREATENING RARE DISEASE. ESTABLISHED IN 2011, THE PHCC PROGRAM HAS ACCREDITED TWO TYPES OF CENTERS. CENTERS OF COMPREHENSIVE CARE (CCC) AND REGIONAL CLINICAL PROGRAMS (RCP). BOTH PHCC-ACCREDITED CCC'S AND RCP'S HAVE DEMONSTRATED A COMMITMENT TO ROVIDING EXPERT-LEVEL CARE BASED ON THE PULMONARY HYPERTENSION DIAGNOSIS AND TREATMENT CONSENSUS GUIDELINES. AT THE END OF 2022, THERE WERE 82 ACCREDITED CENTERS NATIONWIDE.


PULMONARY HYPERTENSION ASSOCIATION REGISTRY (PHAR): ACCREDITED PHCC'S HAVE AN OPPORTUNITY TO PARTICIPATE IN PHAR AS BOTH A QUALITY IMPROVEMENT PROCESS AND A RESEARCH ACTIVITY. A PATIENT REGISTRY IS AN ORGANIZED SYSTEM THAT COLLECTS DATA TO EVALUATE CERTAIN OUTCOMES FOR A POPULATION OF PEOPLE WITH A MEDICAL CONDITION AND IS A USEFUL TOOL FOR QUALITY IMPROVEMENT OF CARE AND FOR RESEARCH OF RARE DISEASES SUCH AS PH. PHAR COLLECTS DATA FROM WHO GROUP 1 PH (PULMONARY ARTERIAL HYPERTENSIONPAH) AND WHO GROUP 4 PH (CHRONIC THROMBOEMBOLIC PULMONARY HYPERTENSIONCTEPH) WHO ARE STARTING EVALUATION AND/OR TREATMENT AT AN ACCREDITED PHCC. DATA IS COLLECTED IN CONJUNCTION WITH ROUTINE PH CARE TEAM VISITS APPROXIMATELY EVERY 3-6 MONTHS. AT THE END OF 2022, THERE WERE 68 CENTERS PARTICIPATING AND 12,323 PATIENTS EVER ENROLLED IN PHAR.
