HELP FAMILIES AND CAREGIVERS OF THOSE WITH ALZHEIMERS AND DEMENTIA RELATED ILLNESSES CARE FOR THEIR LOVED ONES. DIRECT SERVICES PROVIDE: SUPPORT, ENCOURAGEMENT, ACCESS TO COMMUNITY RESOURCES & TECHNIQUES TO HELP MEET CHALLENGES OF CARING FOR SAID LOVED ONES.

Part 3 - Line 4a
ASSIST FAMILIES WITH LONG TERM CARE PLANS INCLUDING DISEASE EDUCATION AND ACTIVITIES OF DAILY LIVING. HOLD WEEKLY SUPPORT GROUPS WITH RESPITE CARE. PROVIDE REFERRALS FOR COUNSELING AND ART CLASSES.
CONTINUED SUPPORT GROUPS FOR FAMILY CAREGIVERS. PROVIDED EDUCATION ABOUT DEMENTIA (PARTICULARLY ALZHEIMER'S) TO ASSISTED LIVING STAFF, DIRECT CARE WORKERS, AND VOLUNTEERS IN THE LOCAL HOSPICE PROGRAM. PROVIDED INFORMATION TO FAMILIES REGARDING ALZHEIMER'S AND OTHER DEMENTIAS. COLLABORATE WITH COMMUNITY PARTNERS TO PROVIDE PROFESSIONAL COUNSELING, RECREATIONAL ARTS AND MUSIC TO CAREGIVERS AND THEIR FAMILIES.
