Describe the Organization's Program Activity:
Part 3 - Line 4a
CASE MANAGEMENT-THE DRC PROVIDES SPECIFIC PROGRAMS THAT FOCUS EXPLICITLY ON IDENTIFYING AND PREPARING HOMELESS INDIVIDUALS FOR RE-HOUSING, PLACEMENT INTO APPROPRIATE HOUSING (SUBSIDIZED AND UNSUBSIDIZED) AND ON-GOING SUPPORTS TO ENSURE LONG-TERM HOUSING PLACEMENT. THESE PROGRAMS ARE REFERRED TO AS HOUSING 1ST PROGRAMS, AND INCLUDE BOTH PERMANENT SUPPORTIVE HOUSING (PSH) AND RAPID RE-HOUSING (RRH.) THE DRC ALSO WORKS WITH MANY HOMELESS INDIVIDUALS WHO HAVE EITHER RECENTLY BEEN RELEASED FROM PRISON, WHERE THEY RETURN TO THE COMMUNITY WITHOUT APPROPRIATELY RECOGNIZED TEXAS ID, OR INDIVIDUALS WHO HAVE LOST THEIR TEXAS ID. IN EITHER CASE, WITHOUT ID AN INDIVIDUAL IS NOT ABLE TO CASH A CHECK, RENT A HOTEL OR APARTMENT, APPLY FOR A JOB, OR ENROLL IN MAINSTREAM BENEFITS. IN SHORT, A TEXAS ID IS CRITICAL FOR AN INDIVIDUALS SURVIVAL AND ENDING HOMELESSNESS. WE WORK WITH THE INDIVIDUALS TO OBTAIN ALL CRITICAL DOCUMENTS (E.G. SSN, BIRTH CERTIFICATE AND ID) TO REESTABLISH THERE FORMAL IDENTITY AND OBTAIN LEGALLY RECOGNIZED TEXAS ID.
