CORNERSTONE AND ITS RELATED ENTITIES OPERATE A CONTINUING CARE RETIREMENT COMMUNITY AND AN ASSISTED LIVING COMMUNITY. THE RELATED ENTITIES ARE LIBERTY HOUSING CORPORATION (THE CORPORATION) AND THE APPALACHIAN CHRISTIAN VILLAGE FOUNDATION, INC. (THE FOUNDATION).
