THE FOUNDATION IS THE SOLE MEMBER OF EMERALD SPRINGS OF YUMA, LLC, WHICH CONSISTS OF A 147-UNIT INDEPENDENT AND ASSISTED LIVING FACILITY LOCATED IN YUMA, ARIZONA.
