CASE MANAGEMENT SERVICES - THESE SERVICES CONSIST OF SEVERAL DIFFERENT PROGRAMS THAT PROVIDE HOME BASED INTERVENTION SERVICES DESIGNED TO ASSIST FAMILIES WITH THEIR CURRENT SITUATION AND IMPROVE THE FUNCTION OF THEIR FAMILY UNIT SERVICES INCLUDE ANY COMBINATION OF INDIVIDUAL/FAMILY THERAPY, ASSISTANCE WITH HOUSEHOLD AND DAILY LIVING SKILLS, PARENT EDUCATION, VISIT FACILITATION, UTILIZATION OF COMMUNITY RESOURCES AND EDUCATION.
HEALTHY FAMILY SERVICES - THESE SERVICE INCLUDE AN ASSESSMENT AND ON-GOING SUPPORT SERVICES TO FAMILIES DURING THE FIRST THREE YEARS OF THE CHILD'S LIFE THAT FOCUSES ON PROVIDING SUPPORT, ENCOURAGEMENT, AND SKILLS IN DEALING WITH THE DAILY STRESSES OF PARENTING.
