TO HELP INDIVIDUALS TRANSITION FROM THE PAIN OF ADDICTION TO THE GRATITUDE OF RECOVERY BY PROVIDING LONG TERM TREATMENT AND COMPREHENSIVE SUPPORT SERVICES.

TRANSITIONS IS A MULTI-LOCATION SOCIAL SERVICE AGENCY THAT PROVIDES THE FULL CONTINUUM OF TREATMENT SERVICES FOR ADULTS WITH SUBSTANCE USE DISORDERS. THE ORGANIZATION IS ACCREDITED BY THE COMMISSION ON ACCREDITATION OF REHABILITATION FACILITIES (CARF).UNDER THE CARE OF THE MEDICAL DIRECTOR AND NURSING TEAM, TRIAGE IS THE FIRST STEP IN THE CONTINUUM OF CARE. IN TRIAGE, INDIVIDUALS ARE MEDICALLY AND CLINICALLY ASSESSED AND PROVIDED WITH CLINICAL RECOMMENDATIONS FOR FURTHER CARE WHICH CAN INCLUDE, BUT NOT BE LIMITED TO, RESIDENTIAL SERVICES, INTENSIVE OUTPATIENT SERVICES OR OUTPATIENT SERVICES. CLIENTS PARTICIPATING IN INTENSIVE RESIDENTIAL TREATMENT RECEIVE INDIVIDUAL, GROUP AND FAMILY EDUCATION AND THERAPY AS WELL AS PARTICIPATE IN OTHER RECOVERY BASED ACTIVITIES TO ENCOURAGE A LIFE OF RECOVERY, HEALTH AND WELLNESS. CLIENTS PARTICIPATING IN INTENSIVE OUTPATIENT (IOP) AND OUTPATIENT SERVICES ENGAGE IN WEEKLY INTENSIVE GROUP THERAPY AND INDIVIDUAL THERAPY WHILE ALSO PARTICIPATING IN COMMUNITY RECOVERY SUPPORT. OTHER SERVICES PROVIDED BY TRANSITIONS INCLUDE MAT, ON-SITE HEALTH CLINIC, MENTAL HEALTH THERAPY, TARGETED CASE MANAGEMENT, CERTIFIED PEER SUPPORT AND RECOVERY HOUSING. THE OVERARCHING OBJECTIVE FOR EVERY LEVEL OF CARE WE PROVIDE IS TO JUMPSTART AN INDIVIDUAL'S LIFE IN RECOVERY. WE DO THIS THROUGH INDIVIDUALIZED TREATMENT PLANNING, PATIENT-CENTERED CARE, EVIDENCED-BASED INTERVENTION STRATEGIES AND A COMPASSIONATE COMMITMENT TO TOP QUALITY CARE.
