THE PURPOSE OF THE ORANGE COUNTY HEALTHY START COALITION IS TO REDUCE INFANT MORTALITY, RACIAL DISPARITIES AND THE INCIDENTS OF PREMATURE AND LOW BIRTH WEIGHT. WE DO THIS THROUGH EDUCATION AND COMMUNITY AWARENESS OF RELATED MATERNAL AND INFANT HEALTH ISSUES, TARGETED OUTREACH TO AND CASE MANAGEMENT OF AT-RISK FAMILIES.
