SELF-RELIANCE, INC. PROVIDES FIVE CORE SERVICES TO INDIVIDUALS WITH DISABILITIES. THE FOLLOWING SERVICES WERE PERFORMED IN FY 2023: 1. ADVOCACY AND LEGAL SERVICES: 120 CONSUMERS SERVED. 2. PEER SUPPORT AND MENTORING: 53 CONSUMERS SERVED. 3. INDEPENDENT LIVING SKILLS AND TRAINING: 469 CONSUMERS SERVED. 4. INFORMATION AND REFERRAL: 2,169 CONSUMERS SERVED. 5. TRANSITION SERVICES: 51 CONSUMERS SERVED. IN ADDITION, 2 CONSUMERS RECEIVED ASSISTIVE TECHNOLOGY AND 28 CONSUMERS RECEIVED HOME MODIFICATION SERVICES.
