REFER TO SCHEDULE O FOR CONTINUATION: CORPORATE FORM OF PUBLIC CHARITY COMMUNITY CHARITABLE FOUNDATION ORGANIZATION DISTRIBUTING GRANTS TO CHARITABLE OR NON-PROFIT ORGANIZATIONS. ALSO AWARDING COMPETITIVE GRANTS, DISTRIBUTIONS, AND SCHOLARSHIPS THROUGH AN APPLICATION AND REVIEW PROCESS. THE FOCUS OF THE AWARDS IS TOWARD EDUCATION, WELFARE ENHANCEMENT, CIVIC PROJECTS, AND COMMUNITY BETTERMENT THROUGH GRANTS TO QUALIFIED ORGANIZATIONS AND COMPETITIVE SCHOLARSHIPS TO INDIVIDUALS IN THE GREATER MID-OHIO VALLEY AND SURROUNDING AREA.

COMMUNITY ACTION GRANTS AND HORNBROOK FUND GRANTS THAT ARE AWARDED VIA A COMPETITIVE GRANT MAKING PROCESS FROM THE FOUNDATION'S UNRESTRICTED AND FIELD-OF-INTEREST FUNDS.


SCHOLARSHIP RECIPIENTS; FUNDS LIST SPECIFIC CRITERIA BY WHICH RECIPIENTS ARE TO BE CHOSEN. THIS INCLUDES FINANCIAL NEED AND ACADEMIC ACHIEVEMENT. SCHOLARSHIP REVIEW COMMITTEES ARE ESTABLISHED BY THE FOUNDATION ACCORDING TO ITS 'SCHOLARSHIP POLICY', AND IN COMPLIANCE WITH H.R.4. MEMBERS TO SUCH COMMITTEES ARE APPROVED BY THE FOUNDATION'S BOARD ANNUALLY AND ASSIST IN THE REVIEW OF CANDIDATES BASED ON THE FUND'S SELECTION CRITERIA AND RECOMMEND RECIPIENTS TO THE FOUNDATION'S BOARD. ALL SCHOLARSHIPS ARE AWARDED WITHOUT REGARD TO RACE, SEX, OR CREED. RECIPIENTS ARE REQUIRED TO SUBMIT EVIDENCE OF POST-HIGH SCHOOL QUALIFIED EDUCATIONAL INSTITUTION ENROLLMENT, AND SUBMIT TRANSCRIPTS OR OTHER EVIDENCE OF ACADEMIC PROGRESS EACH YEAR.


GRANTMAKING FROM ALL OTHER RESTRICTED FUNDS, SPECIAL CAMPAIGNS AND GRANTS RECEIVED BY THE FOUNDATION AND DISTRIBUTED BY THE FOUNDATION FOR THEIR INTENDED PURPOSES.
