FOOD PANTRIES: IN 2021, ATTENDANCE AT THE GREENWICH AND COSSAYUNA FOOD PANTRIES REACHED AN ALL-TIME HIGH SERVING OVER 14,700 INDIVIDUALS CFC PANTRIES PROVIDED FOOD ACCESS FOR NEARLY 100 HOUSEHOLDS EVERY WEEK OVER 250,000 LBS. OF FOOD AND GOODS DISTRIBUTED 92 NEW HOUSEHOLDS VISITED CFC PANTRIES THROUGHOUT 2021


FRESH FOOD COLLECTIVE (FFC):IN 2021, FFC PARTNERED WITH 48 LOCAL FARMS AND PRODUCERS OVER 130,000 LBS. OF FRESH FOOD RECOVERED AND DISTRIBUTED IN 2021 $150,000 DIRECTED TO OUR LOCAL AGRICULTURAL ECONOMY IN 2021 FFC SERVED OVER 1,500 INDIVIDUALS AND DISTRIBUTED 12,795 LBS. OF FOOD AT OUR MOBILE DISTRIBUTION SITES IN 2021. THOSE SITES INCLUDED THE KINGSBURY SENIOR CENTER, CEDARS SENIOR LIVING COMMUNITY, STICHMAN TOWERS AND CRONIN HIGH RISE. CFCS FARM-2-LIBRARY PROGRAM SUPPORTED 8 LIBRARIES INCLUDING ARGYLE, WHITEHALL, PEMBER, SALEM, CAMBRIDGE, SCHUYLERVILLE, ROCKWELL FALLS AND CORINTH LIBRARIES. WE DISTRIBUTED 35,533 LBS. OF FREE, FRESH FOOD TO LIBRARIES IN 2021.


COMMUNITY HEALTH:- FOOD FARMACY IN 2021, CFC COMPLETED THE THIRD ROUND OF FRESH FOOD FARMACY IN PARTNERSHIP WITH HUDSON HEADWATER HEALTH NETWORK (HHHN). OUR COMMUNITY HEALTH TEAM WORKED WITH 42 DIABETIC OR PRE-DIABETIC INDIVIDUALS WHO RECEIVED BI-WEEKLY PRODUCE BOXES OVER 4 MONTHS, 7,000 LBS.+ OF FRESH PRODUCE WAS PROVIDED TO PARTICIPANTS FRESH FOOD FARMACY EXPANSION: IN PARTNERSHIP WITH HHHN, CFC WILL EXPAND TO 4 FQHC IN THE REGION AND GROW TO 100 PARTICIPANTS FOR A 24-WEEK INTENSIVE FOOD AS MEDICINE PRODUCE PRESCRIPTION PROGRAM IN 2022.- CDPHP & GLENS FALLS FARMERS MARKET MORE THAN 2,800 CDPHP INDIVIDUALS VISITED THE CFC MARKET STAND INCLUDING 340 NEW CDPHP MEMBERS VISITING AT LEAST ONCE CFC HOSTED 5 MONTHLY MARKET PROMOTION DAYS WITH CDPHP HIGHLIGHTING DENTAL AND BEHAVIORAL HEALTH, CDPHP CYCLE, AND SEED STARTING WITH CCE WARREN COUNTY. CFC DIRECTED OVER $27,000 TO OUR LOCAL FOOD ECONOMY THROUGH CDPHP AND NOURISH NY DAIRY VOUCHERS.- NEW COMMUNITY HEALTH PROGRAMS LAUNCHED IN 2021 FIT AND FRESH IN PARTNERSHIP WITH THE SARATOGA YMCA-BATTENKILL BRANCH OFFERING FREE NUTRITION, COOKING, AND FITNESS CLASSES TO PROGRAM PARTICIPANTS. PROGRAM TO CONTINUE IN 2022. EMPLOYEE WELLNESS WITH HUDSON HEADWATERS HEALTH NETWORK WHICH PROVIDED EACH HHHN EMPLOYEE (900 IN TOTAL) ACROSS THEIR NETWORK WITH LOCAL NY PRODUCTS. THIS DIRECTED $20,000 TO THE LOCAL FOOD ECONOMY. MEDICARE MONTHLY SUPPLEMENTAL PRODUCE BOXES: DESIGNED AND DISTRIBUTED 80 SUPPLEMENTAL PRODUCE BOXES TO CDPHP MEDICARE MEMBERS- OVERALL COMMUNITY HEALTH IMPACT 5,700+ CULINARY AND NUTRITIONAL RESOURCES PROVIDED ACROSS ALL PROGRAMS 450+ REFERRALS PROVIDED TO PARTNER ORGANIZATIONS AND PROGRAMS 52 INDIVIDUALS PROVIDED WITH INTENSIVE HEALTH COACHING 60+ LOCAL PRODUCERS AND FARMS SUPPORTED
