BEHAVIOR MANAGEMENT SERVICES (BMS)DESIGNED TO MATCH CHILDREN AND YOUTH WITH TRAINED ADULTS WHO PROVIDE 1:1 BEHAVIORAL ASSISTANCE IN THEIR HOME, SCHOOL, AND/OR COMMUNITY AND TEACH A VARIETY OF LIFE SKILLS TO PROMOTE SUCCESS. PARENTS ARE PROVIDED TOOLS AND SUPPORT FOR MORE EFFECTIVE PARENTING. BMS IS DESIGNED TO SERVE CHILDREN IN THE LEAST RESTRICTIVE ENVIRONMENT POSSIBLE AND MAY BE UTILIZED TO TRANSITION CHILDREN FROM HIGHER LEVELS OF CARE TO THEIR HOME, SCHOOL, AND COMMUNITY; MAKING IT POSSIBLE FOR CHILDREN TO MAINTAIN IN THE LEAST RESTRICTIVE SETTING.COMPREHENSIVE COMMUNITY SUPPORT SERVICES (CCSS)THE PURPOSE OF COMPREHENSIVE COMMUNITY SUPPORT SERVICES (CCSS) IS TO COORDINATE AND PROVIDE SERVICES AND RESOURCES TO CONSUMERS/FAMILIES NECESSARY TO PROMOTE RECOVERY, REHABILITATION, AND RESILIENCY. CCSS IDENTIFIES AND ADDRESSES THE BARRIERS THAT IMPEDE THE DEVELOPMENT OF SKILLS NECESSARY FOR INDEPENDENT FUNCTIONING IN THE COMMUNITY, AS WELL AS STRENGTHS WHICH MAY AID THE CONSUMER OR FAMILY IN THE RECOVERY OR RESILIENCY PROCESS. CCSS ACTIVITIES ADDRESS GOALS SPECIFICALLY IN THE FOLLOWING AREAS: INDEPENDENT LIVING, LEARNING, WORKING SOCIALIZATION, AND RECREATION. CCSS ALSO INCLUDE SUPPORTING A CONSUMER AND FAMILY IN CRISES SITUATIONS AND PROVIDING INDIVIDUAL INTERVENTIONS TO DEVELOP OR ENHANCE A CONSUMER'S ABILITY TO MAKE INFORMED AND INDEPENDENT CHOICES. PSYCHIATRIC REHABILITATION GROUPS (PSR) THE GOAL OF OUR PSYCHIATRIC REHABILITATION PROGRAM IS TO ASSIST INDIVIDUALS IN LEARNING THE SKILLS NECESSARY TO MAINTAIN INDEPENDENT LIFESTYLES IN THE COMMUNITY. SKILLS TRAINING IS OFFERED IN GROUP SETTINGS, ASSISTING THE PERSONS SERVED TO ACHIEVE THEIR GOALS OF CHOICE IN THE FOLLOWING COURSE/AREAS: -EDUCATION GROUPS ASSIST IN UNDERSTANDING AND MANAGING MENTAL ILLNESS -PRE-VOCATIONAL SKILLS TRAINING -COMMUNITY LIVING -COMMUNITY ACTIVITIES AND OUTINGS -SOCIALIZATION -LEISURE EDUCATION/AWARENESS -DUAL DIAGNOSIS EDUCATION INDIVIDUALS SERVED ARE REFERRED TO THE PSR PROGRAM BY THEIR COMMUNITY SUPPORT SPECIALIST TO ADDRESS A WIDE VARIETY OF SKILLS TRAINING AND EDUCATION, WHICH WILL FOCUS ON THE INDIVIDUALS' RECOVERY PROCESS. GOALS ARE IDENTIFIED BY THE INDIVIDUAL AND TRACKED FOR COMPLETION THROUGHOUT THE 90 DAY CYCLE.COUNSELING AND PSYCHOTHERAPYLICENSED THERAPISTS ARE ABLE TO WORK WITH INDIVIDUALS, COUPLES, FAMILIES, AND GROUPS. INDIVIDUAL SEEKING THERAPY MAY REQUEST THIS SERVICE DURING INTAKE OR AT ANY TIME DURING ENROLLMENT AT AGAVE HEALTH. SESSIONS ARE PROVIDED BY APPOINTMENT WITH THE ASSIGNED CLINICIAN. MEDICATION SUPPORT SERVICES (MSS)AGAVE HEALTH'S NURSING PROVIDES MEDICATION SUPPORT SERVICES (MSS). THE NURSES ARE PART OF A MULTIDISCIPLINARY TEAM THAT ENSURES CONSUMER'S CONSISTENT USE OF THEIR MEDIATIONS, ASSESS THEIR RESPONSE TO MEDICATIONS, AND PROVIDE EDUCATION REGARDING THEIR ILLNESS AND MEDICATIONS. MSS IS A COLLABORATIVE PROCESS INVOLVING THE CONSUMER AND THE CONSUMER'S MULTIDISCIPLINARY TEAM. PSYCHOTROPIC EDUCATION WILL BE PROVIDED ON ALL MEDICATIONS AND WILL ALWAYS BE AN ONGOING PART OF THE PROCESS. CONSUMERS WILL CONTINUALLY BE TRAINED TO ASSUME INDEPENDENT RESPONSIBILITY FOR MEDICATION MAINTENANCE. SUBSTANCE ABUSE SERVICES SUBSTANCE ABUSE SERVICES ARE PROVIDED AT AGAVE HEALTH IN BOTH INDIVIDUAL COUNSELING AND GROUP FORMAT TO ADDRESS THE NEEDS OF THOSE CONSUMERS DEALING WITH SUBSTANCE USE RELATED ISSUES. CLINICAL STAFF MEMBERS AT AGAVE HEALTH CAN ALSO PROVIDE THE ADDICTION SEVERITY INDEX (ASI) AN ASSESSMENT WHICH EVALUATES FOR SUBSTANCE ABUSE AND/OR MENTAL HEALTH NEEDS. THE ASSESSMENT CAN BE A USEFUL TOOL IN LEARNING MORE ABOUT ONESELF AND SUBSTANCE USE ISSUE. THE ASSESSMENT CLINICIAN WILL PROVIDE HELPFUL TREATMENT RECOMMENDATIONS THAT A CONSUMER CAN INCORPORATE IN HIS OR HER OWN RECOVERY.INTENSIVE OUTPATIENT PROGRAM (IOP)PROVIDES A TIME-LIMITED, MULTI-FACETED APPROACH TO TREATMENT SERVICE FOR INDIVIDUALS WHO REQUIRE STRUCTURE AND SUPPORT TO ACHIEVE AND SUSTAIN RECOVERY. IOP MUST UTILIZE, AT A MINIMUM, A RESEARCH-BASED MODEL AND TARGET SPECIFIC BEHAVIORS WITH INDIVIDUALIZED BEHAVIORAL INTERVENTIONS.PSYCHIATRIC MEDICATION PRESCRIPTION SERVICES AGAVE HEALTH OFFERS ACCESS TO PSYCHIATRIC MEDICATION MANAGEMENT FOR INDIVIDUALS SEEKING MEDICATION TO ASSIST IN THEIR RECOVERY AND MANAGEMENT OF THEIR PSYCHIATRIC SYMPTOMS. AGAVE HEALTH OFFERS SERVICES WITH DOCTORS SPECIALIZING IN PSYCHIATRIC MEDICATION PRESCRIPTION. INDIVIDUALS SEEKING MEDICATION PRESCRIPTION SERVICES MAY REQUEST AN APPOINTMENT WITH A PRESCRIBING PROVIDER BY CONTACTING THE AGAVE HEALTH FRONT DESK.TREATMENT FOSTER CARE (TFC)CHILDREN AND TEENS WITH SERIOUS EMOTIONAL AND BEHAVIORAL PROBLEMS RECEIVE INTENSIVE TREATMENT SUCH AS THERAPY, SKILLS DEVELOPMENT, PSYCHIATRIC CARE, AND ACCESS TO COMMUNITY RESOURCES WHILE LIVING IN A FAMILY SETTING WITH SPECIALLY-TRAINED AND LICENSED FOSTER PARENTS THROUGHOUT THE STATE.
